£27,099 per annum for 36 hours per week working across Frimley Park Hospital and the community services based within Camberley.

About the role
Surrey Heath Reablement Service forms part of the Surrey Heath Integrated Care Team, working with local health and social care providers to enable residents to improve their levels of independence so they can return or remain at home.
We currently have an exciting opportunity to join the team as a Team Leader. Focusing on our hospital discharge and Integrated Care pathways you will work flexibly and divide your time between Frimley Park Hospital and the community services based within Camberley.
You will join a team of dedicated and experienced Team Leaders and work together to improve the service for the benefit of the local community.
