It is an exciting time to become part of Community Services at Runnymede Borough Council, as we further develop joint working initiatives with health and social care partners as well as neighbouring Councils, supporting an integrated approach to health and care and those most in need within our communities.

 

Homesafe Plus is the hospital discharge scheme managed and coordinated by Runnymede Borough Council on behalf of the four Borough Councils that make up the North West Surrey area of Surrey Heartlands Clinical Commissioning Group.  The service provides a single point of access to a range of Borough services, enabling health and social care professionals to make a single referral in support of a patient awaiting hospital discharge, across the four Borough areas. 

 

The use of Homesafe Plus has grown year on year to a position of 1,000+ referrals received in 2020/2021, and the ambition is to continue to develop both its use and the range of services that can be accessed through this single point of access.  You will lead in the coordination and development of Homesafe Plus, working closely with partner Borough Councils and health and social care partners to develop the single point of access, further integrate the service into wider health and social care pathways, increase the services on offer, and to widen its use across the system.  You will also lead the compilation of monitoring information supplied by each of the partner boroughs, demonstrating the value and impact of the service. 

 

To be successful in this role, previous experience of working in a health, social care or other wellbeing related service is required, as well as an understanding of the role that prevention services provided by Borough Councils can play in supporting both residents directly and  the wider health and care system. You will need to be proficient in the use of ICT, both in the use of Microsoft Office as well having an aptitude to learn other software platforms. Good analytical and problem solving skills are also key to this role.

 

This North West Surrey Alliance (Health & Care Alliance) funded post forms part of a wider project led by the partner borough councils including support for the planning of hospital discharges and the provision of step down accommodation for those medically fit to be discharged from hospital but unable to return home immediately.  As a result, you will be required to work with other stakeholders from across the health and care system on a regular basis, and therefore experience of working in partnership with others and advanced written and verbal communication skills are required.
