It is an exciting time to become part of Community Services at Runnymede Borough Council, as we further develop joint working initiatives with health and social care partners as well as neighbouring Councils, supporting an integrated approach to health and care for those most in need within our communities.

 

Homesafe Plus is the hospital discharge scheme for the North West Surrey area of Surrey Heartlands Clinical Commissioning Group.  The service provides a single referral point to access a range of borough services for example: community alarms, meals at home, housing, home improvement agency.  The use of this service has grown significantly, and the ambition is to continue to develop both its use and its range of services.

 

The successful candidate will lead the coordination and development of Homesafe Plus, working closely with partner borough councils, health, and social care partners to further integrate the service into wider health and social care pathways, increase the services on offer, and to widen its use across the system.  You will also lead the compilation of monitoring information supplied by each of the partner boroughs, demonstrating the value and impact of the service. 

 

To be successful in this role, experience of working in a health, social care or other wellbeing related service is desirable, as is an understanding of what role borough prevention services can play in supporting both residents and the wider health and care system. You will also need to be proficient in the use of ICT, both in the use of Microsoft Office as well having an aptitude to learn other software platforms.

 

This North West Surrey Alliance (Health & Care Alliance) funded post forms part of a wider project led by the partner borough councils including support for the planning of hospital discharges and the provision of step down accommodation for those medically fit to be discharged from hospital but unable to return home immediately.  As a result, you will be required to work with other stakeholders from across the health and care system on a regular basis, and therefore experience of working in partnership with others and advanced written and verbal communication skills are required.
