Integrated Reablement Worker
The starting salary for this position is £21,943 per annum based on a 36 hour working week.
An opportunity has arisen for a 12-month secondment/fixed term post to support the Farnham area. This exciting opportunity enables you to support integrated working between health and social care.
You will be working in individual's homes; promoting people to regain daily living skills and general well-being for those who have experienced mental or physical health conditions.
Joining us as an Integrated Reablement Worker (IRW), you'll be doing something that really matters. Working autonomously, you'll have the responsibility for individual, sensitive needs of those who require support.
It will be your job to provide support with all daily living tasks, including medication and mobility. You'll be there to ensure dignity, choice and control is maintained for those who feel like they have lost it from their lives. They'll look to you for support and guidance, a helping hand and most importantly, the development of their reablement programme - allowing them to continue with their lives. The IRW role will support not only the in-house reablement staff but will assist in supporting and setting up skills gain programmes with our Collaborative Reablement Domiciliary Care Partners.
Part of this role will require you to oversee and guide more junior staff members. You will need to have the ability to undertake and contribute to the assessment and review of individuals, this includes accurate record keeping within our IT databases.
Vocational qualifications Level 2 or equivalent in a relevant field with practical knowledge of service user group needs, or equivalent experience is essential.
It is also crucial that you have a valid UK driver's licence with vehicle and business insurance, as you'll be required to travel to visit people in their own homes. Above all you'll understand how important maximizing independence is to the people who use our services.
